To help assist us in meeting these goals, having the following information sent prior to their appointment is extremely helpful:

Medical records related to the referring problem, including doctor’s notes, SOAPs

Bloodwork

Imaging (radiographs, ultrasound, including radiology reports)

Any outside laboratory results (including cytology or biopsy reports)

Any additional information that you think may be useful regarding the case
